Welcome to the website content survey.  This survey will help us gather information and content for your website project.  The survey is simply a form with questions and fields to upload documents and images, as well as to provide YouTube or Vimeo links you may wish to see on your website.

Please be as detailed and thorough in this survey.  The more information you provide, the sooner we will deliver a website draft that meets your approval.

If you are uploading documents that provide content, please label or name the files to correspond to the page you wish to see the content.

When uploading images, please ensure they have a minimum resolution of 1500 pixels.

Describe how the website would serve the people who visit, not YOU. Examples: Provide information and updates, be informative about topics or social issues, enable online applications for services or programs, login to an online account, book services, purchase products,etc.
